Instructions For Completing the Form :
This form helps immediate supervisor to perform his basic roles as follows:
1.
Reviewing Employee's skills and competencies in the light of the immediate supervisor's comments for the performance period (last year).
2.
Reviewing Employee Performance for the performance period. Period should be reviewed against planned objectives stated at the beginning of the year in (last year Performance Meeting) plus any assignments given to him during the course of the year.
Note: Immediate supervisor is obliged to comply with the basic guidelines contained in the Performance review guidelines.
